Two-Way Voice for Jan, Family Vaults & Power of Attorney Delegation
Today’s focus centered on accessibility for patients managing extreme fatigue, tools for multi-generational family caregivers, and eliminating anxiety around when hospital records were last synchronized.
🎙️ Hands-Free Voice Input & Stabilized Speech for Jan
- Hands-Free Speech Dictation: Patients experiencing joint pain, tremors, or severe physical exhaustion can now tap a microphone icon and dictate directly to Jan rather than struggling to type.
- Soothing Voice Stabilization: Resolved browser audio quirks so Jan consistently speaks in a warm, comforting voice across page reloads without defaulting to robotic system alerts.
- Proactive Section Tour Guide: As patients navigate between sections (Wearables, Labs, Medications, Vitals, and Sharing), Jan proactively explains what the screen does and suggests relevant questions.
👥 Multi-Generational Family & Caregiver Support
- The Family Vault Plan: Introduced a shared household subscription allowing up to four profiles under one primary account, drastically reducing costs for families where multiple members manage chronic conditions.
- Caregiver & Medical Power of Attorney (POA) Tools: Family advocates can now be invited with tailored roles—including one-time specialist links, view/edit caregiver permissions, and formal Medical Power of Attorney documentation verification.
- 1-Tap Profile Switcher: Caregivers can seamlessly switch between an elderly parent's records and their child's health profile in the top navigation bar without logging out.
🏥 Hospital Connection History & Public Experience
- "Last Imported On" Timestamps: Added a clean ledger card displaying all connected health networks (like Epic MyChart) with exact dates and record tallies so patients know when their charts were refreshed.
- Direct App Access: Replaced placeholder links on our marketing portal with direct launch access to the active application at
vitalmosaic.org.
- Verified Clinical Statistics: Aligned our clinical blueprints with NIH, CDC, and Dysautonomia International research documenting POTS prevalence, diagnostic delays, and neuro-immune cognitive fatigue.
The 7-Question AI Safety Gate & Trauma-Informed Medical Ergonomics
Medical software should heal, not panic. We established our binding Psychological Safety Charter to protect patients from late-night health anxiety spirals.
🛡️ Radical Calm by Design
- The 7-Question Safety Gate: Formalized a mandatory checklist for every AI feature before release, verifying context over quick answers, transparent uncertainty bounds, and human clinician follow-ups.
- Non-Alarmist Lab Telemetry: Eliminated aggressive red alert styling for chronic baseline fluctuations, replacing them with grounding amber and slate tones to reduce autonomic triggers.
- Mindful Friction: Introduced gentle confirmation pauses before users review potentially sensitive or catastrophic differential records.
500-Page Clinical Fax Parsing & Zero-Hallucination Provenance
Patients often receive their medical history in giant stacks of faxed PDFs. We built the engine that structures these binders in minutes.
📄 Ingestion with Complete Traceability
- 15-Organ-System Longitudinal Heatmap: Categorized scattered lab panels into clinical domains (immunologic, autonomic, endocrine, hematologic, etc.).
- Click-to-Source Provenance: Every extracted biomarker and diagnosis links directly back to the highlighted snippet in the original medical PDF, giving clinicians 100% confidence in data accuracy.
- 2-Minute Specialist Briefs: One-tap export summarizing the top 3 appointment priorities, active medication reconciliations, and recent lab deltas.
Easy Mode Architecture: Designing for Brain Fog & Autonomic Calm
When you're having an active flare, traditional dense medical charts are unusable. We created an instant 1-tap view designed specifically for tired eyes and cognitive fatigue.
🌿 Guided Mode (Easy Mode)
- Large Typography & High Contrast: Soft contrast themes designed to prevent ocular migraine and photophobia triggers.
- Rule of Three: Restricts action choices to maximum 3 high-priority cards per view to protect executive function.
- Smart Pillbox Circles: 1-tap daily dose completion rings for morning, midday, evening, and bedtime routines.
Patient Data Sovereignty, Account Record Purge & Google Healthcare NLP
We believe patient privacy is an absolute human right. Today we shipped full patient data sovereignty and integrated Google Cloud Healthcare Natural Language Processing.
🔒 Absolute Ownership & Traceability
- One-Tap Complete Account Purge: Implemented a secure, irreversible data deletion engine. Patients can purge their entire medical vault at any moment with zero residual backups on our servers.
- Google Cloud Healthcare NLP Integration: Connected medical entity extraction to parse complex clinical notes, converting dense diagnostic prose into structured timelines.
- Patient Sharing Controls: Added granular permission toggles allowing patients to generate secure view-only links for their specialists.
Dual-Domain Launch Architecture: Moving the Live Portal to VitalMosaic.org
As our clinical footprint expanded, we separated our public informational front door from our encrypted clinical web application.
🌐 Scalable Cloud Routing
- Dedicated Application Domain: Moved the active React web app to the root of
vitalmosaic.org, keeping vitalmosaic.us dedicated to patient guides, provider education, and leadership resources.
- Expanded Epic FHIR Scopes: Stabilized SMART on FHIR OAuth permissions to securely query both
DocumentReference and Procedure resources across regional hospital networks.
- Daily Health Diary: Added the daily reflection diary into the Health Tracker so patients can record subjective flare observations alongside numeric vitals.
Production Hosting Setup & Visual Identity Integration
Transitioned our infrastructure to multi-site Firebase App Hosting, established production routing, and integrated our unified logo and design system.
🛠️ Engineering Foundations
- Multi-Target Firebase Hosting: Configured automated deployments for web applications and informational front-doors.
- Radial Mosaic Branding: Integrated the circular mosaic radar and ECG pulse wave vector identity across all navigation headers and modals.
- Closed Beta Access Controls: Provisioned early invitation and preview pathways for initial patient testing.
Separating RAIdar into an Interactive Clinical Assistant
Evolved RAIdar from a passive visualization heatmap into an interactive, multi-turn clinical reasoning companion.
🧠 Interactive Medical Reasoning
- Standalone Assistant Workspace: Built a dedicated consultation panel where patients can ask plain-language questions about their lab trajectories.
- Multi-Factor Cross Correlation: Empowered RAIdar to cross-reference medication changes directly against subsequent shifts in inflammatory and metabolic biomarkers.
Bridging the Hospital Gap: Early SMART on FHIR & HealthKit Ingestion
Broke down the barrier between hospital electronic health records and daily biometric wearable telemetry.
🌉 Unified Health Bridges
- Native Health Bridge: Designed the bridge architecture linking Apple HealthKit and Android Health Connect to capture resting heart rate, orthostatic vitals, and sleep data.
- FHIR R4 Normalizer: Built the translation engine that maps proprietary hospital data into standardized clinical observation schemas.
The Codebase Genesis: Migrating from AI Studio to Production Repository
Migrated the early Google AI Studio prototype into a standalone, enterprise-grade React, TypeScript, and Firebase application repository.
🚀 Professional Software Foundation
- Production Codebase Setup: Initialized the repository with 212 files, full TypeScript typing, and Vite bundler pipelines.

- Server-Side AI Pipeline: Restored Gemini 2.5/3.8 Flash as the core reasoning engine backing clinical synthesis.
The Spark: Solving My Own Healthcare Maze
On July 28, 2026, Melissa Ruth began creating what is now known as Vital Mosaic with the goal of solving her own problems.
After cancer and immunotherapy treatment, Melissa found herself managing care across multiple doctors, labs, tests, and diagnoses scattered across multiple electronic health records systems. In order to address her pain, navigate complex symptoms, and bring order to her medical journey, she turned to Gemini in hopes it could help synthesize the disconnected pieces of her health.
It wasn’t until August, when multiple treating physicians saw what she had created and asked how their other patients could use it, that she realized this was something that needed to exist for everyone.
Weaving software development late into the night between her daily work, frequent medical appointments, and procedures, Melissa continued to build—driven by the clear conviction that Vital Mosaic was no longer just for her, but for every patient and caregiver walking the chronic illness maze.
